Please read these Terms & Conditions ("Terms") carefully before using www.mercedesbaez.com (the "Website") or engaging with the services offered by Mercedes Baez, RN, MSN ("I," "me," or "my"), a sole proprietor providing aesthetic and wellness services including but not limited to PRP hair restoration, neurotoxin treatments, dermal fillers, and other aesthetic procedures.
By accessing this Website, scheduling an appointment, making a payment, or communicating with me electronically, you agree to be bound by these Terms. If you do not agree, please do not use this Website or my services.
1. Services provided — scope of practice
Mercedes Baez, RN, MSN is a licensed registered nurse and aesthetic practitioner operating as a sole proprietorship. Services are performed within the applicable scope of practice under the laws of the state(s) in which services are rendered and under appropriate medical oversight or collaborative agreements as required by applicable law.
This Website and the services offered herein are for aesthetic and wellness purposes only. Nothing on this Website constitutes medical advice, a diagnosis, or a treatment plan for any medical condition. You are encouraged to consult with your primary care physician or a specialist before initiating any aesthetic treatment, particularly if you have existing medical conditions, are pregnant, or are taking prescription medications.
I do not provide emergency medical care. If you believe you are experiencing a medical emergency, call 9-1-1 or go to your nearest emergency room immediately.
2. Not a substitute for professional medical care
Aesthetic services, including injectable treatments and PRP therapy, are not substitutes for comprehensive medical care. You should not disregard or delay seeking medical advice based on anything you read on this Website. Always consult a qualified healthcare provider regarding any medical concerns and before stopping, starting, or modifying any treatment or medication.
3. Informed consent & assumption of risk
Prior to receiving any service, you will be required to review and sign a separate informed consent document specific to each treatment. That document will describe the nature of the procedure, potential risks and side effects, alternatives, and what to expect before, during, and after treatment.
By proceeding with any service, you acknowledge and accept that all aesthetic procedures carry inherent risks, including but not limited to bruising, swelling, infection, allergic reaction, asymmetry, and in rare cases, more serious complications. Results are not guaranteed and may vary based on individual factors including but not limited to age, skin type, health status, and lifestyle.
You represent that you have disclosed all relevant medical history, current medications, allergies, and prior aesthetic treatments, and that all information you provide is truthful and complete. Providing inaccurate or incomplete information may affect your safety and the outcome of your treatment.
4. Appointments, bookings & payments
Appointments are scheduled through this Website and require payment at the time of booking. By completing a booking and submitting payment, you agree to the following:
All appointment fees are due at the time of scheduling unless otherwise agreed in writing.
Payment is processed securely through our third-party payment processor (e.g., Squarespace Payments or Stripe). I do not store your payment card information directly.
You represent that all payment information you provide is accurate, complete, and that you are authorized to use the payment method submitted.
Appointment fees are non-refundable unless cancellation is made in accordance with the cancellation policy below.
5. Cancellation & rescheduling policy
I understand that schedules change. To respect the time of all clients and ensure appointment availability, the following policy applies:
Cancellations made at least 48 hours before your scheduled appointment may be eligible for a full refund or credit toward a future appointment.
Cancellations made less than 48 hours before your appointment may result in forfeiture of your deposit or booking fee.
No-shows without prior notice will be charged the full appointment fee.
To cancel or reschedule, please contact me at hello@mercedesbaez.com as soon as possible.
I reserve the right to modify this policy at any time. The policy in effect at the time of your booking will apply.
6. Electronic communications & SMS text messaging
By providing your mobile phone number and/or email address, you consent to receive electronic communications from Mercedes Baez, RN, MSN, including via email and SMS text message, for the following purposes:
Appointment scheduling, confirmations, and reminders;
Post-treatment follow-up and care instructions;
Practice updates and informational communications; and
Promotional or educational content related to services offered (you may opt out at any time).
Message frequency: Message frequency varies based on your appointment schedule and interactions with our practice.
Message and data rates: Standard message and data rates may apply depending on your mobile carrier plan.
Opt-out: You may opt out of SMS communications at any time by replying STOP to any text message. You may opt back in by contacting us directly. Opting out of marketing messages will not affect appointment-related communications unless you request otherwise.

Important: Electronic communications, including text messages, are not a substitute for medical consultations and should not be used to report urgent medical concerns, adverse reactions, or emergencies. If you experience an adverse reaction following a treatment, seek in-person medical attention or call 9-1-1 immediately.
SMS opt-in data and consent will not be shared with third parties for marketing purposes.
7. Privacy & health information

Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded under applicable law, including any applicable professional liability standards. My professional liability insurance covers claims arising from clinical services rendered within my scope of practice. These Terms do not alter, waive, or limit any rights you may have under applicable healthcare or consumer protection laws.
14. Indemnification
You agree to indemnify and hold harmless Mercedes Baez, RN, MSN from and against any claims, damages, losses, liabilities, and expenses (including reasonable attorney's fees) arising out of or related to: (a) your use or misuse of this Website; (b) your breach of these Terms; (c) any false, inaccurate, or incomplete information you provide; or (d) your violation of any applicable law or the rights of a third party.
15. Governing law & dispute resolution
These Terms are governed by and construed in accordance with the laws of the state in which services are primarily rendered, without regard to its conflict of law provisions.
In the event of any dispute arising from or related to these Terms or the services provided, the parties agree to first attempt to resolve the matter informally by contacting me at hello@mercedesbaez.com. If informal resolution is unsuccessful, disputes shall be resolved through binding arbitration administered under the rules of the American Arbitration Association (AAA), except that either party may seek injunctive or equitable relief in a court of competent jurisdiction to prevent irreparable harm. You agree to bring any claim in your individual capacity only and not as a plaintiff or class member in any class action proceeding.
16. Force majeure
I shall not be liable for any failure or delay in performing obligations under these Terms where such failure or delay results from causes beyond my reasonable control, including but not limited to acts of God, natural disasters, pandemics, government orders, or other events outside my reasonable control. I will make reasonable efforts to notify you and resume services as soon as practicable.
